Peninsula Health Plan (UPHP) is completely
voluntary. If
you choose to communicate with the health plan via
the Internet by sending us electronic mail (e-mail),

You can
browse the Web site anonymously.
UPHP
does not collect any information when you visit the
Web site.

Collect When You Communicate With Us via the

UPHP is committed to protecting the privacy of
personal information that you provide to the health
plan via the Internet.

which we periodically review and update.
If you choose to communicate with us via the
Internet, we may request personally identifiable
information from you necessary for us to process
your request for information, respond to questions,
or deliver a product or service.

address, e-mail address, or UPHP member number.

be used by our employees and contractors who need to

computer systems. UPHP
cannot guarantee the confidentiality of Internet
communications. Use
of the Internet is at your own risk.

UPHP has taken steps to make all information
received from our online site as secure as possible
against unauthorized access and use.
All
information is protected by our security measures,
which we periodically review and update.

during your Internet communications with us.
