Peninsula Health Plan (UPHP) is completely
voluntary.
If you choose to communicate with the
health plan via the Internet by sending us
electronic mail (e-mail), you should read the
statement below.
You can browse the Web site anonymously.
UPHP does
not collect any information when you visit the

Collect When You Communicate With Us via the

UPHP is committed to protecting the privacy of
personal information that you provide to the
health plan via the Internet.
All
information is protected by our privacy
policies, which we periodically review and
update.
If you choose to
communicate with us via the Internet, we may
request personally identifiable information from
you necessary for us to process your request for
information, respond to questions, or deliver a
product or service.
Personally
identifiable information is any information that
can be used to identify you, such as your name,
physical address, e-mail address, or UPHP member
number. When
you send information to us using e-mail, the
information may be used by our employees and
contractors who need to know the information to
respond to your request and are permitted or
required by law to do so.
We keep
some of this information for a period of time,
as determined necessary by us. |

There are several risks that you should consider
before using e-mail to communicate with UPHP:
E-mail can be circulated, forwarded, and
stored in numerous paper and electronic files;
e-mail can be intercepted, altered, forwarded,
or used without authorization or detection;
e-mail senders can easily misaddress an e-mail;
backup copies of e-mail may exist even after the
sender deletes the e-mail; employers and online
services have the right to inspect e-mail
transmitted through their systems; e-mail is
easier to falsify than handwritten or signed
documents; an electronic message can be used to
introduce viruses into computer systems.
UPHP
cannot guarantee the confidentiality of Internet
communications. Use
of the Internet is at your own risk. |

UPHP has taken steps to make all information
received from our online site as secure as
possible against unauthorized access and use.
All
information is protected by our security
measures, which we periodically review and
update.
We use security technology to protect you during
your Internet communications with us.
This
security includes server authentication, privacy
using encryption, and data integrity.
Your
personal information is protected from
unauthorized access by a secure firewall and
security infrastructure.
During
transmission of information using one of our
Internet services requiring identification and a
password, your privacy is protected by Secure
Socket Layer (SSL), a minimum of 128-bit
encrypting software.
We employ extensive physical,
electronic, and procedural safeguards to
maintain the security of your information.

health plan also conducts periodic reviews of
its Web security to ensure that its electronic
security safeguards remain up-to-date and
