Upper Peninsula Health Plan (UPHP) Medicaid members carry two identification cards.

 

◙    mihealth Card

 
  • Permanent, plastic card
    		•
  • Possession of card does not verify coverage.
    		•
  • Members may order replacement cards by calling the Michigan Medicaid
         Beneficiary Help Line at 1-800-642-3195.
    		•

     

    ◙    UPHP Card

     
  • Member identification number and assigned primary care provider
    		•
  • How to bill UPHP
    		•
  • The card does not verify Medicaid or UPHP eligibility.
    		•
  • Members may order replacement cards by calling Customer Service at
       1-800-835-2556.
    		•
