At Upper Peninsula Health Plan, we’re proud to share our new commercial featuring one of our Care Coordinators and a member from our Upper Peninsula Health Plan (UPHP) MI Coordinated Health (HMO D-SNP) program. The video highlights what care coordination means to us—building trust, listening closely, and ensuring every member feels supported. Our Care Coordinators are licensed health care professionals who connect members to the services they need, from medical care and transportation to home help and community support. We focus on the whole person because every part of life impacts health.
Upper Peninsula Health Plan (UPHP) MI Coordinated Health (HMO D-SNP) is a health plan that contracts with both Medicare and Michigan Medicaid to provide benefits of both programs to enrollees. Enrollment in UPHP MI Coordinated Health depends on contract renewal.
