About UPHP Medicaid

Medicaid

The Upper Peninsula Health Plan (UPHP) is a managed care organization licensed by the State of Michigan to provide Medicaid medical services. You do not need to pay for medically necessary services that are covered on the contracted fee schedule and coordinated with your primary care provider (PCP).

Are you eligible for Medicaid?

Find out if you are eligible for Medicaid by applying at the Michigan Department of Health and Human Services (MDHHS) MI Bridges website or by filling out an application at your local Department of Health and Human Services (DHHS) office. After you enroll in Medicaid

UPHP is the only Medicaid Health Maintenance Organization (HMO) in the Upper Peninsula. Once you enroll in Medicaid you will most likely be auto-enrolled into UPHP. If you are not auto-enrolled, call Michigan ENROLLS toll-free at 1-888-367-6557 and ask to be enrolled into UPHP.
