About UPHP Medicaid
The Upper Peninsula Health Plan (UPHP) is a managed care organization licensed by the State of Michigan to provide Medicaid medical services. You do not need to pay for medically necessary services that are covered on the contracted fee schedule and coordinated with your primary care provider (PCP).
Are you eligible for Medicaid?
Find out if you are eligible for Medicaid by applying at the Michigan Department of Health and Human Services (MDHHS) MI Bridges website or by filling out an application at your local Department of Health and Human Services (DHHS) office. To find an office near you, click on your home county on the DHHS locator map. For further information on the Medicaid program, visit the MDHHS website.
