Healthy Michigan Plan
The Healthy Michigan Plan provides health care benefits to Michigan residents at a low cost so that more people can have health coverage. Upper Peninsula Health Plan (UPHP) provides Healthy Michigan Plan coverage and benefits through the Michigan Department of Health and Human Services (MDHHS).
Are you eligible for Healthy Michigan Plan?
Find out if you are eligible for Healthy Michigan Plan by applying at the MDHHS MI Bridges website or by filling out an application at your local Department of Health and Human Services (DHHS) office. After you enroll in Healthy Michigan Plan
UPHP is the only Medicaid Health Maintenance Organization (HMO) in the Upper Peninsula. Once you enroll in the Healthy Michigan Plan you will most likely be auto-enrolled into UPHP. If you are not auto-enrolled, call Michigan ENROLLS toll-free at 1-888-367-6557 and ask to be enrolled into UPHP.
