About UPHP Healthy Michigan Plan
Healthy Michigan Plan
The Healthy Michigan Plan provides health care benefits to Michigan residents at a low cost so that more people can have health coverage. Upper Peninsula Health Plan (UPHP) provides Healthy Michigan Plan coverage and benefits through the Michigan Department of Health and Human Services (MDHHS).
Are you eligible for Healthy Michigan Plan?
Find out if you are eligible for Healthy Michigan Plan by applying at the MDHHS MI Bridges website or by filling out an application at your local Department of Health and Human Services (DHHS) office. To find an office near you, click on your home county on the DHHS locator map. For further information on the Healthy Michigan Plan program, visit the Healthy Michigan Plan’s website.
