Health Risk Assessment

The UPHP Medicaid Health Risk Assessment (HRA) is a set of questions about your health behaviors that you can answer every year. It asks about your current habits, like how much you exercise and what kind of food you eat.  It helps your provider better understand your health and what you can do to get and stay healthy.
