Upper Peninsula Health Plan (UPHP) offers wellness programs to help members reach their health goals.

UPHP offers the following wellness programs to members:

Who is eligible?

Members ages 18 years and older who use tobacco products are automatically enrolled. Members may opt out of this program at any time by calling UPHP Quality Management at 1-800-835-2556.

What is the program?

The goal of this program is to help people cut back or stop tobacco use.

What does the program help with?

  • Motivation to quit
  • Tips on how to get started with a quit program
  • Medication to stop tobacco use

This program supports members who are:

  • Chewing tobacco
  • Smoking cigarettes
  • Using e-cigarettes
  • Vaping

Members who use tobacco are eligible for two programs:

  • Quitting Smoking and Tobacco Use counseling: UPHP covers visits with your doctor to talk about quitting tobacco.
  • Michigan Tobacco QuitLink: 1-800-QUIT-NOW (1-800-784-8669)
    The Michigan Tobacco Quitlink is a free service. It is for members thinking about quitting, not yet ready to quit, or have already quit. UPHP Medicaid and Healthy Michigan Plan (HMP) members can call us at 800-835-2556 to learn if you could earn a gift card after calling the Quitlink for four sessions.
  • Truth Initiative EX Program – FREE access to the EX Program. A personalized digital program to help whether you smoke, dip, vape, or chew tobacco. By completing the EX Program, you could earn a $100 gift card.

Being sent home after a stay in a nursing home can be overwhelming. UPHP offers a Transition of Care Program to help members during this time.

It is proven that people who see their primary care provider within 30 days of returning home are much more likely to do well.

Who is eligible?

Members age 18-64 years who have been discharged home after a stay in a nursing home are automatically enrolled. Members may opt out of this program at any time by calling UPHP Quality Management at 906-225-4654.

What is the program?

You will receive a card in the mail after your nursing home stay. A UPHP clinical coordinator will then contact you by phone. We can offer:

  • Help with understanding instructions you received at discharge
  • Help with scheduling a visit with your regular care provider within 30 days

Who is eligible?

UPHP members of all ages looking to maintain or achieve a healthy weight are eligible for the Nutrition Therapy Wellness Program. You will need to ask your provider for a referral to see a registered dietitian for nutrition therapy visits.

What is the program?

The Nutrition Therapy Wellness Program provides visits with a registered dietitian who can:

  • Teach ways to eat a healthy diet
  • Help start an exercise program
  • Support your goals to keep a healthy routine

Where does the program take place?

A local hospital or health department.

How many visits do you get?

You can attend up to two visits.
