Program Overview

Upper Peninsula Health Plan Pharmacy is excited to enhance collaboration with pharmacy providers through our Alternative Payment Model (APM) program.

The goal of the APM program is increased partnership with pharmacies to enhance Medication Therapy Management Services provided to our members. The APM will help acknowledge the impact pharmacies and their staff have on our members’ medication adherence and other health outcomes, while offering a payment model that rewards pharmacies for promoting health care innovation and improved quality.
