General

For members who can transport themselves or have a friend, family member, or neighbor to take them, they can fill out a UPHP Mileage Reimbursement Request Form.

  • If a friend, family member, or neighbor is transporting you, they must fill out the driver section in full.
  • If it is the first time that the friend, family member, or neighbor has transported a UPHP member, they must fill out the UPHP NEMT Driver Attestation  for UPHP to process the reimbursement.

For members who have no vehicle, friend, or family member to transport them, UPHP will arrange transport using the least cost-effective means. This can be via bus, cab, or volunteer driver. The member should contact UPHP at least 5 business days prior to the appointment.

  • You must have active UPHP coverage at the time of the appointment.
  • You must be seeing a provider in UPHP’s network or an out-of-network provider with an approved prior authorization. You can view UPHP’s provider directory at UPHP.com.
  • Your medical service must be for a UPHP-covered appointment or service such as a visit with your primary care provider, chiropractor, physical therapist, outpatient procedure, or for picking up durable medical equipment (DME) or pharmacy prescriptions.

UPHP transportation reimbursement is set by the Michigan Department of Health and Human Services commonly known as MDHHS.  This can change without notice to members or volunteer drivers. You can see the current MDHHS Fee schedule here: Non-Emergency Medical Transportation (michigan.gov)

UPHP is a Heath Maintenance Organization (HMO) meaning that we have a network of providers members must utilize. Your referring provider can request prior approval for you to be seen out of network. This is called prior authorization. You must have an approved prior authorization on file in order for UPHP to assist with transportation.

Once your doctor submits this to UPHP, UPHP has 14 days to make a decision.

You can attach proof of your appointment to the form.

  • Examples: Visit summary, discharge or admission paperwork, a note from a provider’s office, or a receipt for a prescription pick up.

UPHP has 30 days to process your reimbursement once we’ve received it at UPHP. Please allow for additional time for the mail.

You can status your reimbursement online at any time on the UPHP Assist portal https://assist.uphp.com

  • If you do not see your reimbursement pending or closed in the portal, this means UPHP has not processed your reimbursement.
  • UPHP reserves the right to deny or reduce reimbursement based on sanction screening or UPHP network rules.
  • Forms must be filled out in their entirety and all receipts must be original, dated, and itemized.
  • If UPHP notices it has been 90 days since we sent a check that has not been cashed, you will receive a letter to your address on file. This must be filled out attesting you did not receive your check and returned to UPHP in order for a new check to be sent out.
  • Ensure your address is correct with MDHHS to ensure you receive your mail. You can update your address by calling MDHHS at 844-464-3447 or MI Bridges at MI Bridges (michigan.gov)

You can sign up for direct deposit by filling out this form Direct Deposit Authorization form. You must complete the form and attach a voided check or statement from your bank.

  • The member will need to be traveling at least 200 miles one way or 400 miles round trip for their appointment to be eligible for meals and lodging.
  • Meal assistance is for the day of the appointment only or for members and one legal guardian.
  • Original, dated, and itemized receipts must be submitted with the reimbursement form.
  • UPHP cannot reimburse meals purchased on a bridge card, or with a food assistance card.
  • UPHP cannot reimburse for any alcohol and tobacco products.
  • All receipts should be attached to the Mileage Reimbursement Request Form.
  • UPHP partners with select lodging and meal programs to provide a direct bill option to reduce the financial burden on the family. When utilizing this option, you must contact UPHP prior to the appointment to request assistance.
  • Depending on your UPHP plan, some of your services are covered through MDHHS instead of UPHP. If your appointment or medication is covered through MDHHS with your green MI Health Card, you can request transportation help at 844-464-3447.

UPHP transportation will reimburse for mileage, lodging and meals for eligibility appointments 90 days from the date of service.

IOS (Apple products)

  1. Open “Notes” and create a new one.
  2. Tap on the Camera button, then tap “Scan Documents”
  3. Place documents in view of the camera.
    1. If your device is in auto mode, your document will automatically scan, otherwise tap the circle to capture the image.
  4. Tap “Save” or add additional scans to the document.

Android (Samsung, Google, Motorola, etc.)

  1. Open the Google Drive App.
  2. In the bottom right, tap “Add”
  3. Tap “Scan”.
  4. Take a photo of the document you’d like to scan.
    1. You can adjust the scan area by taping “Crop”.
    2. You can scan another page by tapping “Add”.
  5. Create your own title.
  6. Save the finished document by taping “Save”.

Transportation

UPHP uses the most cost-effective means that meets the members medical needs. We will look to arrange your transport through Bus, cab or volunteer driver. If there is a medical reason you are unable to use the most cost-effective means, or you need a wheelchair transport, your medical provider can fill out the UPHP Medical Needs Form.

Make your appointment first, then call or submit your request online at least five business days before your appointment date.

For urgently scheduled (same or next day) appointments, we will try our best to accommodate. At a minimum, we can provide mileage reimbursement.

If the UPHP transportation coordinators arrange a volunteer driver, the volunteer driver will call you to set up a pickup time. The Transportation Reimbursement Request form is sent to the driver. They will give this to you when you go into your appointment to get it signed or provide proof of the appointment. The volunteer driver will send this back to UPHP to receive mileage reimbursement for transporting you.

Yes, but bus, cabs, and volunteer drivers are NOT allowed to sign anyone out of the hospital after they have had a procedure done. Sedation requires someone to attest they will keep watch over you for 24 hours.  If a member needs to be signed out of the hospital after a procedure, they will need a friend or family member to accompany them.

If you need to cancel or reschedule a transport, call transportation as soon as possible at 1-800-835-2556. If we do not answer, please leave a voicemail message.

Once you call UPHP transportation and UPHP approves the transport, you should call the bus in your area and schedule your ride with them to get your pickup time. Transportation calls in all buses the Friday before the following week to let the company know that UPHP is paying for the ride.

If you ride Marquette Area Transit Authority, UPHP will send you transportation vouchers. These are only good for the date listed on the voucher. If your appointment is canceled or rescheduled, please contact UPHP and they will advise you on what to do. Throw away the transportation voucher as you will not be able to use it for any other date than what is on the voucher.

You will receive a Transportation reimbursement request form in the mail. Ensure your provider signs this and mails it back to UPHP.
