UPHP Assist Member Portal User Guide

Account Creation

Go to https://assist.uphp.com/ and select “Create a new account” under New User Login on the right-hand side of the page. From here select Create Member Account under “Members” located on the left-hand side. The next page will look like this:

Fill out the following:

  1. First Name
  2. Last Name
  3. Member ID
  4. Date of Birth
  5. Preferred Email Address (this will be the username of the account)
  6. Confirm Email Address
  7. Postal Code

Once these are filled out, click Next at the bottom.

On this page select a security question from each of the dropdowns on the left and answer them. Also select the Language, Country, and Time Zone on the right. Click Next afterward.

The end-user agreement is shown on the next page. Check the checkbox once this has been read to agree. Click Save on the bottom right to proceed.

Once registration has been completed, an email will be sent to the email address on the account with a link to create a password.

Creating a Password

When creating a password, the following criteria must be met:

  • A minimum of eight characters.
  • Contains at least one numeric character.
  • Contains at least one uppercase and one lowercase letter.
  • Include at least one punctuation symbol.
  • Must not be easily guessable.
  • Must not be the same as the last five passwords.
  • Must not contain a repeating pattern of characters or words.

Forgot Password

If a member has forgotten their password, go to the login page and select “Forgot Password?” above the Password textbox.

The next page shows the username on the account and gives different options to reset the password. Members can either:

  • Get a password reset link sent to the email tied to the account or,
  • Answer the security questions on the account

Forgot Username

If a member has forgotten their username, they can reach out to the Upper Peninsula Health Plan at 1-800-835-2556.

Verification Code

After logging in, members will get a code sent to their email. This code can be copied and pasted into the field shown here:

If a new code is needed, select the Resend new Code button.

Here members can access the New Message section, Sitemap, Contact Us page, and profile settings at the top right.

New Message: A page that allows members to submit a question regarding their claims, benefits, PCPs, pharmacy claims, or questions about the portal.

Sitemap: Lays out the pages of the member portal.

Contact Us: Brings members to the Contact Us page on the UPHP website.

Profile Settings: Members can change their password or security questions, their contact preferences, and view their profile, as well as logoff.

Members can view their claim history, benefits, documents, and use the provider search. They can also request a Primary Care Provider (PCP) change and print ID cards.

Within the banner at the top left the UPHP Assist logo can be used to bring members back to the homepage.

Buttons

The buttons shown here are as follows:

  1. Review My Policy: Allows members to view their benefit policy.
  2. View Claims: Allows members to view their claim history.
  3. Print ID Card: Members can print and request their ID Card.
  4. Provider Network: A provider search.
  5. Documents: Plan specific documents and forms.
  6. Assessments: Brings the member to the MyUPHP Portal website to view and complete assessments.
  7. Mailbox: Brings the member to their inbox.
  8. Message My Care Coordinator: Brings the member to the MyUPHP Portal website where they can view and send messages to their Care Coordinator if they have one assigned to them.

Quick Links

Links to various websites will be added at the bottom left of the homepage for easy access.

From the Claims tab in the white banner, members can view both Claims History and Prescription History.

Claims History

Members can view and search claim history under Claims>Claim History. Once on the page, members can view all their claims up to six years back. Each claim shows the provider, the date of service, the claim status, and the amount paid by UPHP. Claims that read “Transportation Default” will have a grey button that reads Claim Summary (shown in the red box below). The Claim Summary is a form that shows more details about the payment.

At the bottom of the page, members can go between pages of claims. There is also an option to print the current page of claims or to download all claims to a spreadsheet. Members can narrow down the search by entering a specific from and to service date in the two calendar fields shown at the top of the page.

To view more information on a claim, members can select the “View Details” button. This will take members to a page that shows all claim lines included in that claim as well as details to each line.

Members can also do the following while viewing the details of a claim:

  • Print
  • Download as PDF
  • Download as Excel CSV File

Arrows are located at the top left and right of the claim view. These can be used to go back and forth between different claims.

Prescription History

Members can view prescription claim history under Claims>Prescription History. Here members can view prescription claim details including:

  • Date prescription (Rx) was filled
  • Drug name
  • Prescriber Name
  • Pharmacy Name
  • Pharmacy Phone
  • Days Supply
  • Quantity

Members can narrow down their search be entering the date the drug was filled or the drug name. They may also print the list of prescription claims or download them at the bottom of the page.

Members may also click on a prescription in the list to pull up additional details (shown below). There is button (in the red box) that can be used to view more information about the drug in question.

View Benefits

Benefits can be viewed under Benefits>View Benefits. On the Benefits page, members can switch between the view of their current benefits or all benefits by using the switch located in the red box shown here:

Benefits show the plan name, the first and last name of the member, their date of birth, member ID, coverage start and end dates, and the group name the plan belongs to.

Members can also view and print out a proof of insurance, as well as view benefit documents using the two red buttons displayed on the page.

At the top of the page, there are two buttons named Benefits and Accumulators. Each button brings the member to the named area on the page.

Accumulators are at the bottom of the page. Please note that accumulators reflect paid UPHP claims in the member portal. Members may also print out the benefits page, which is located above the Accumulators. If members see a negative amount while viewing accumulators, please contact Customer Service at 1-800-235-2556.

Print ID Card

Members can print their ID card by going to Benefits>Print ID Card. Here, members can view their current ID card, request a new one, or print one out. Members can only request one new ID card per day.

Plan Documents

The Documents page, located under Documents>Documents, allows members to download documents related to the plan they are in. Members can also use the search bar to find a specific document.

Policy Documents

View My Care Plan

Members can view their Care Plan by going to Documents>View My Care Plan. From there, members will be taken to the MyUPHP Portal website to view their current Care Plan.

Provider Network

The Provider Network page, located under Provider>Provider Network, brings members to the provider search on the UPHP website. Members can search for providers based on the plan they are enrolled in.

Primary Care Provider

Members can view and request a new Primary Care Provider (PCP) under Provider>Primary Care Provider. The PCP page shows the member’s PCP and their phone number. It also shows all requested PCPs and their name and specialties.

For members who would like to request a new PCP, select the “Request New PCP” button under Providers>Primary Care Provider. Once on the page, members can look up PCPs by using the following fields:

  • Doctor first and last name
  • State (either Michigan or Wisconsin)
  • City
  • Doctor Specialty
  • Zip Code

Once the fields are filled out, click search at the bottom of the page. The page shown on the next page will open.

Search Results

This page shows the location, phone number, name and credentials of the PCPs that fit the fields searched. To request a PCP within the list, hover over the PCP you’d like to request and a red button that says “Request as PCP” will show on the far left. Members can also download a PCP directory based on their search at the bottom of the page, or have the list emailed to them.
